Direct Support Professional
QUALITY IMPROVEMENT MANAGER
UNA LOCATION:Portage Industries or MASCO
IMMEDIATE SUPERVISOR: Chief Operating Officer
SUMMARY: This position is responsible for coordinating quality management and process improvement efforts, including development, implementation, education, data collection and analysis, to ensure UNA delivers the highest standards of client care, safety, outcomes and community integration.
ESSENTIAL DUTIES:
· Hold team meetings for each individual to help them develop an individualized goal based on their needs and preferences.
· Train staff to provide quality of care for each individual based on their goals.
· Discuss issues with management team, make recommendations for improvement, and propose solutions to individuals’ desires for goals in both their day program and in the community.
· Suggest technology based on the needs of each individual receiving services.
· Provide training to staff on improved processes.
· Improve adult day services for individuals specifically with complex needs.
· Build adult day services program and increase the capacity of individuals we serve.
· Create training materials to help staff work with the individuals receiving adult day services.
· Attending conferences or seminars to stay current on technology trends available to individuals with disabilities.
· Work with supervisor to develop a tracking system for goals to ensure they are achieved by each individual.
· Reevaluate goals and suggest changes based on need.
· Improve person centered outcomes including achievement of unique goals, building skills for daily living and integration into the community.
· Assure program compliance with any grant requirements.
· Other duties as assigned.
QUALIFICATIONS and CHARACTERISTICS:
· High school diploma or GED.
· 3-5 years’ experience in working with individuals with disabilities or related field
· Ability to travel locally.
· Must maintain attention to detail.
· Problem-solver with innate ability to analyze data and make decisions based on data
· Reliable, consistent team player with solid interpersonal skills
· Working knowledge of Microsoft Office products (Word, Excel, PowerPoint, Office)
· Experience with grant administration preferred.
PHYSICAL REQUIREMENTS: Sufficient sight and hearing to carry out responsibilities. Must be able to sit, stand, and work at a computer for long periods of time. Must be able to walk production floor and meet with employees. The position may require periodic lifting, bending, stooping, stretching, pulling and pushing, and must occasionally lift up to 50 lbs.
OTHER REQUIREMENTS:
· Valid driver’s license.
· Pass criminal background check.
· Pass pre-employment drug screen.
· Pass pre-employment physical exam.
· Complete all DODD training requirements.
· Complete CPR, AED, and first aid training.
FLSA: Salaried Exempt
WORK SCHEDULE: Mon-Fri, 8 hours per day, and any additional time that is required to perform the job.
WORK LOCATION: Home office varies between home, office, and community settings. Occasional remote work is available. Home office can be Portage Industries or Masco.

Direct Support Professional (DSP)- All locations
Job Description:
SUMMARY: This position is responsible for day-to-day care of our clients, ensuring the ISP is followed. Helps clients integrate into the community, and assists with production work inside the workshop environment. Transports clients to and from workshop using company vehicles.
ESSENTIAL DUTIES:
· Ensures the health, safety, and security of program participants by adhering to all related policies and procedures
· Provides supervision to program participants in an adult workshop, or other day habilitation or community setting according to board policies/procedures and individualized service plans
· Assists program participants with personal care needs in areas of function such as eating, drinking, using the toilet, positioning, walking, using a wheelchair, transferring and other assessed needs
· Implements behavior support plans with emphasis on positive and proactive measures
· Maintains accurate and complete production, attendance, incident repot, behavioral and individual plan documentation
· Maintains quality control standards as set forth by the production supervisor on all piece work or other production
· Works cooperatively with other adult services employees to develop and implement individual service plans
· Transports clients to and from company locations and community events using company vehicles
· Ensures and maintains harmonious and effective relationship with other employees, program participants, parent/guardians, and state and local community service agencies
· May be required to restrain individuals who present risk of harm to self or others
· Other duties as assigned
QUALIFICATIONS and CHARACTERISTICS
· High School Diploma or GED

· Must be able to adapt to changes
· Reliable punctuality and attendance
PHYSICAL REQUIREMENTS: Must be able to sit, stand, stoop, stretch, bend and lift up to 50 pounds. Must be able to push clients in wheelchairs if necessary. May be required to physically restrain client if behavior is a danger to themselves or others. Must be able to occasionally use computer. Eyesight capabilities must be sufficient to perform these functions. Hearing must be adequate to perform these functions.
OTHER REQUIREMENTS:
· Must possess valid drivers license
· Must pass criminal background check
· Must pass pre-employment drug screen
· Must pass pre-employment physical exam
· Must obtain and maintain current First Aid and CPR/AED Certificates
· Complete all DODD training requirements
